TO THE NEW PATIENT

DAY I

1. STEP ONE:

To prevent a delay in processing you as a patient, please bring an insurance form/card so we can verify your coverage. This can save you time on your next visit.

2. STEP TWO:

We request you complete a health history questionaire. Please fill in all areas; including who referred you to our office. Sign the last page.

3. STEP THREE:

Tubbs Chiropractic will evaluate your medical history and recommend an examination to determine if care in our office may be of benefit to you. X-rays may be necessary to determine the cause of your health problems.

4. STEP FOUR:

At this time, we will discuss your insurance coverage and schedule you for your next appointment to discuss your X-ray findings with the doctor. You will be given a report of findings on your next visit. The doctor will inform you as to the "cause" of your problem.

DAY II

1. STEP ONE:

The doctor will review your X-rays with you. If the doctor feels treatment will be of benefit, he will discuss a treatment plan at this time. Treatment will continue until maximum correction has been obtained.
